Chancellor Cole from Spirit Tracks, the Hero's Spirit from TP, OoT Link's mother, and the golden wolf from TP. And the two on the side of those four are Dark Link and Oni/Fierce Deity Link.

I don't think it ever actually showed her, she was just mentioned in the backstory. Her look seems to be based on Zelda's mourning robe from TP (which might be intended to play into the theory that Link and Zelda are brother and sister in OoT, making Link's mother the queen).
