In World 5-Fortress #2, jump straight up to wedge yourself between the exit pipe and the right side of the screen. Keep jumping to climb up the wall, and small Mario will get "smashed" and lose a life when he reaches the ceiling. Super Mario can duck and jump to get the same result. (To safely unwedge yourself, walk to the left against the wall until you drop down.)
